A situation where difficulty, risk, or pain has shifted from ordinary hard into the truly nasty range. In engineering slang, nightmare mode usually means bad conditions stacked together in exactly the wrong way.
In plain English: The worst hard painful version of a technical situation.
Example: "The rollback entered nightmare mode once they discovered the old clients, stale cache, and schema drift would all interact during recovery."
